- Middle management kept hiring people even though upper management was already decided to fire the team they were hiring for ( multiple teams were reduced ) - Higher management seems clueless on what would help the product - Product owner did not have a clear vision how things should work - People only wanted to get their job done ASAP and did not care about best practices - Huge amounts of tech debt
